Q. What is the principle of the frame over the foot intended to correct the deformity ?
1. Controlled collapse.
2. Ligamentotaxis.
3. Distraction histogenesis.
4. Fractional distraction.
Answer –
‘Fractional differential distraction’ is the principle
behind the frame (JESS – Joshi’s External Stabilisation System) used here to
correct CTEV (Club Foot). The difference in rate of distraction between medial
and lateral side of foot helps achieve correction.
Ligamantotaxis is used in various fractures so that
ligamantous stretching is used to reduce the fracture or maintain the fracture
length and stability following the reduction. Eg. Use of external fixator
devices for distal radius fractures.
Distraction histogenesis is the process resulting in de novo
new bone regeneration as result of longitudinal stretching of callus. Eg
Illizarov’s method used for various indications like infected nonunion, limb
lengthening , deformity correction etc.
‘Controlled cancellous collapse’ is the principle behind DHS
(Dynamic Hip Screw) application for
inertrochanteric fractures.
